This is not exactly the sort of problem likely to be run into very often.
1) Checklist entry.
2) Use Preview buttton.
3) Hit Add Name button (by mistake).
4) Add Name to Checklist window appears.
5) Not wanting to add a name, hit Cancel.
6) Kaboom!

I did not explore what would happen if I did try to add a name.

Roy,
I think that problem has now been encountered twice.
I believe I had it once, and may have reported it. I was just clicking stuff automatically, and got out of sequence with where I was.
Then again, I may not have reported it because when I am on autopilot, i have trouble replicating the error.
